- 2 Certified copies of Graduation Certificate and the Grade List
(Please show the original certificate to UGM officer
on the day you re-register yourself) - 2 Certified copies of complete Transcript

(to be compiled in the student’s portfolio – click here to download) - Fill in the enrollment form (click here to download)
- Original Receipt of Admission & Tuition Fee payment
