Send your printed application confirmation sheet before application deadlines to the Secretariat of International Programme Faculty of Medicine
Universitas Gadjah Mada, attached with the following documents*):

• Certified copy of Institutional TOEFL/IELTS/MUET certificate (Overseas Applicants only)
• Certified copy of personal ID card
• Size 4x6 coloured photograph (3 pieces)
• Bank/Agent receipt of application fee payment
• Certified copy of SPM result (Malaysian applicants only)
• Certified copy of graduation certificate (if any)
• Certified copy of most recent academic results (Overseas Applicants only)
• Certified copy of academic reports (Indonesian Applicants only)

Incomplete application will not receive full consideration.

Note:
*) Faculty of Medicine reserves the rights to refuse an applicant to sit in the Admission Test if he/she fails to show the original documents required by the time of verification day.
